When visiting our web site in order to enroll in one of our programs, make a purchase, register to use certain services, enter a contest, respond to a survey, or update your information, you may be asked to provide certain personally identifiable information about yourself (“Personal Information”), such as your name, email address, physical address, telephone number, date of birth, or mother’s maiden name. For enrollment and purchases, we require a name, physical address, telephone number, credit or debit card or related checking account number and card expiration date. We use Personal Information to perform business functions, such as serving our members and other customers, developing new products and services, fulfilling orders and contacting you when necessary, such as when a delivery may be delayed or when an ordered product is out of stock.
